Patient Terms & Conditions

I, the undersigned, hereby acknowledge and accept the following practice regulations:

Please note that if you are not an SA resident you will be required to sign an indemnity form.

  • As the self-declared person responsible for this account (irrespective of whether I am the main member or a dependant on a medical aid), I accept that I am solely responsible to settle this account.

  • I acknowledge that my account must be paid on the day of my appointment.

  • I will notify the practice immediately, in writing, should any of my personal or medical aid information change. 

  • A cancellation fee will be charged unless appointments are cancelled more than 24 hours in advance.

  • Through this document I have been informed that when I enter into consultation with the FAC, I will be giving them my personal information that may be protected by data protection legislation, including but not only, the Protection of Personal Information Act, 2013 (POPI) and that the FAC will take all reasonable steps to protect my personal information.

  • In case of a complaint or dispute regarding the care provided by either the FAC, it's practice staff or locum associates - patients must undertake to embark on a course of formal mediation before litigation is pursued. Please click on the link below to read more about the pre-mediation clause and procedures for dispute resolution.

  • I agree to not communicate with the FAC on medical matters via sms – all such correspondence must go via email.

  • Please arrive 10 - 15 minutes before your appointment to allow time to fill out our patient registration forms, including a brief family medical history, and sign the patient's terms and conditions agreement.  You can read through the agreement by clicking the link below.

  • Please bring your doctor’s referral/ request form.

  • Please bring any previous scans relevant to your current pregnancy or gynaecological examination as well as the results of any relevant blood tests.

  • Children under 10 years are not permitted in the scan room.

  • Please allow two hours for your visit to the fetal assessment center. This will usually be adequate time for your examination, consultation, report and photos to be prepared for you to take home

  • Please switch off your mobile phone upon entry to the scan room as these can interfere with the medical equipment. Mobile phone calls are not permitted during the examination.

  • Appointments are an average of 45 minutes long. However, depending on how the baby is lying, appointments can take up to 2 hours.  Please plan your day accordingly.

  • When booking online please note that no booking is confirmed until you receive confirmation by e-mail. Confirmation will be sent within 24 hours during weekdays.

IMPORTANT:  Please read through and familiarise yourself with the information contained in the link below on prenatal testing, as provided by to the South African Society of Obstetricians & Gynaecologists (SASOG).  You will be required to complete and sign this form upon arrival at the FAC. By proceeding with an appointment booking you acknowledge that you have read this document and understand its contents.
